Shot in the head and recovering from brain surgery, Delta Force soldier Thomas Nelson wakes up in a military hospital unable to remember who he is. Faced with going through recovery alone, with no family that he's aware of, he is recruited by a top secret government agency. Project Specter is tasked with eliminating targets they deem to be a threat to the United States. Their goal is to turn Nelson into a super assassin named Matthew Cain.

Cain, still reeling from the attack on his girlfriend, gets even more suspicious of Project Specter when they send him to Russia to find Dmitri Kurylenko, a man he has already secretly killed. After another attempt on his life, he soon learns of the agency's involvement in keeping his memory from him and sets to meet with a man who seems to know everything.

Franks finds himself in a bind, and enlists the help of his friend, Brett Jacobs, to help alleviate the problem. Barberio's Butchers, a small-time gang with big-time aspirations, is looking to put a hurting on the pawn shop owner. It's up to Jacobs to make sure that doesn't happen.

He settles on a former CIA agent named Paxton Phillips. They are all hopeful that Phillips will blend into the group as easily as Haley once did. Unfortunately, they are not so lucky. Phillips does not necessarily share the same beliefs as the rest of the team, and he instantly clashes with Vincent’s men.

As they uncover a name, they also discover his connections to Vincent’s organization. Is Vincent secretly trying to finally take the Silencer out of the picture? Or someone that Recker previously had a conflict with?

The battle with Wilson Ames continues to rage. Jacobs worries about the safety of Tiffany and her parents. CIA Black Ops agent John Smith is on his latest assignment in London. Little does he know - it'll be his last. Another menacing force has entered the city. The Tri-State Scorpions have migrated from New Jersey and are looking to make the city their own. Recker and the team have plans to stop them, but it will come at a cost. After one of the leaders of the Scorpions gets in a shootout with Haley, he is transported to a local hospital where Mia is working. The Scorpions take over the hospital, causing it to go on lockdown. After receiving a troublesome text message that indicates Mia is in some type of danger, Recker must find a way to get into the hospital and save her before it's too late.

Hall is lured into a trap, then rescued by an old acquaintance. That same acquaintance also helps Hall get his next job. It’s nothing but a simple blackmail case. Only in this case, there’s nothing simple about it. Is anybody actually who they appear to be? And have we seen the last of James Rankin? Or is he the mastermind behind the latest case, once again?

Recker finally reaches his boiling point and confronts Jones about his lack of progress in finding Agent 17. The professor, feeling guilty about not locating him, admits to a secret he's been hiding. As Recker is trying to find Agent 17, Mia launches her own investigation into her friend's death, frustrated that the killer still hasn't been brought to justice. Her lack of experience causes a major mistake - one that may cost the life of both her and Jones. Recker must not only find Agent 17, but save the lives of his friends before it's too late.

Alex Parker is back and, along with her partner, Jim Cole, still on the trail of a rogue CIA agent. Their pursuit takes them all over the world.
